All students are required to submit a completed Report of Medical History, proof of immunity to vaccine-preventable diseases. All students entering the University for the first time and those returning after an absence of one or more semesters are required to submit a completed health examination (Entrance Medical Record). Students will not be allowed to register for classes if their enrollment and medical forms have not been returned by the announced deadline date. The University reserves the right to deny registration to any student or prospective student who, in the judgment of the officials of the University Health Service, is suffering from a condition of illness that would be inimical to the health of others. In such cases, the recommendation of the University Health Service is final.
